Why these are combined

Retatrutide hits GLP-1, GIP and glucagon receptors; cagrilintide hits the amylin receptor. Both are in late-stage trials on their own, but no study has combined them. The logic is that adding a fourth pathway to an already triple-agonist drug should extend the metabolic effect, but that's a guess from mechanism, not a result.

Mechanism & pharmacokinetics

Retatrutide (a GLP-1/GIP/glucagon triple agonist) and cagrilintide (an amylin agonist) both carry roughly week-long half-lives by design, so the pharmacokinetics line up cleanly for once-weekly co-dosing even though nobody has run them together in a trial. Mechanistically they'd be hitting four separate receptor systems at once (GLP-1, GIP, glucagon, and amylin), each individually linked to appetite or energy-expenditure effects — the logic for combining them mirrors CagriSema's, just with a triple agonist standing in for semaglutide. That also means four times the receptor systems with no combined safety data, rather than two.

Safety: No safety data exists for the combination. Expect additive GI side effects and the same lean-mass-loss concern as other multi-agonist drugs. Both compounds are still investigational, so any combined use is off-label and unsupervised by definition.
