Pentosan Polysulfate

An FDA-approved medication for interstitial cystitis that restores bladder protective layer, with risk of vision changes from long-term use.

Evidence category: Regulator / guideline. This describes the source category, not approval or certainty.

Regulatory statement in source: FDA-approved.

Pentosan polysulfate sodium (Elmiron) is an FDA-approved oral medication for relief of bladder pain or discomfort associated with interstitial cystitis, approved September 26, 1996. It is the most studied oral medication for interstitial cystitis, with evidence from over seven randomized trials involving 500+ patients (Grade B evidence). In one multicenter randomized controlled trial of 148 patients, 32% on pentosan polysulfate showed significant improvement compared with 16% on placebo. The mechanism involves restoration of the damaged glycosaminoglycan (GAG) protective layer on the bladder urothelium. However, post-marketing surveillance since approval has identified serious visual risks including progressive pigmentary maculopathy associated with long-term exposure that threatens vision.

Benefits reported in the imported source

  • Symptom relief in interstitial cystitis, shown in randomized trials.
  • Addresses underlying GAG layer damage mechanism

Risks and limits

  • Post-marketing surveillance identified serious visual risk: progressive pigmentary maculopathy with long-term use
  • Vision-threatening effects were not recognized in initial clinical trials
  • 21-year pharmacovigilance analysis documents previously unreported serious adverse events

Source bottom line: Pentosan polysulfate is an FDA-approved medication for interstitial cystitis with demonstrated efficacy, but long-term use carries a risk of serious vision-threatening pigmentary eye changes identified through post-marketing surveillance.

Pharmacokinetic context

Historical half-life estimate: 20–27 hours. FDA label, Elmiron: plasma radioactivity half-life about 20-27 hours across the 300/450 mg dose range (reflects total drug-related material, not necessarily unchanged parent) Population/route context and estimates may differ. This is not a schedule or a verified human dosing rule.
